Who's the person with the car in the palm of their hand? The 車 here actually comes from 電車 (train), so this person is the train conductor. Think about it — the conductor handles things like checking tickets, making announcements, and giving signals to the driver, so everything that happens on the train is quite literally in their hands… er, palms.
Note that these days, even many Japanese people get the 車掌 mixed up with the 運転士 (driver). However, the 車掌 is not the person driving the train — they're more like the train's overall supervisor, in charge of managing passengers and overseeing safety.
